Services encompass skilled nursing, physical
therapy, occupational therapy, speech
therapy, medical social worker, and home
health aide. All employees are experienced
and carefully screened. Medicare: free in
home evaluations for qualified Medicare
recipients. Available seven days a week,
twenty four hours a day. During service and
after discharge, an R.N. on call can answer
questions regarding patient care and changes
in condition.
Skilled Nursing
– Experienced Registered Nurses, assisted by
Licensed Vocational Nurses, implement a
comprehensive, individualized plan of care
in conjunction with the client’s physician.
Observation and assessment of a patient’s
condition with acute or chronic illness,
teaching and training, catheter care, wound
care, nutritional needs, oxygen,
rehabilitative care to include bowel and
bladder training and medication management.
Physical Therapist
– Licensed Physical Therapists assist
patients rehabilitate and improve gait,
transfers, range of motion, provide
therapeutic intervention for pain control,
instruct caregivers in methods of assisting
patients in transfers and gait, enhance the
functional abilities of the patient or
increase the ease of caring for the
patient.
Occupational Therapist
– Licensed Occupational Therapists help
patients regain their ability to perform
bathing, dressing toileting, cooking and
eating. Assist patients who require
instruction in one handed techniques,
impaired cognition, perception or awareness
of body parts. Instruction provided for
joint protection, energy conservation
techniques, and may recommend adaptive
equipment to enhance their functional
ability.
Speech Therapist
– Licensed Speech Therapists assist patients
with decreased ability to express and/or
receive communication, with impaired
cognition and/or memory, ability to phonate,
and dysphagia with swallowing instructions.
Medical Social Worker
– Licensed Social Worker for referrals to
community resource planning, inadequate
financial resources, stress management, and
counseling for long-range planning and
decision making.
Home Health Aide
– The Home Health Aide can assist or
complete personal care services to include
bathing, grooming meal preparation, eating,
oral hygiene, skin and/or nail care,
toileting and elimination, ambulation, and
exercises, light housekeeping, washing
clothes, and grocery shopping. |
